Empowering Navigators in Nottinghamshire

Helping people access the services they need

Challenge

Local Design Teams within the Community Transformation programme identified a need to create a digital place for people in the community and professionals in health, social care and voluntary support services. This place ‘to start’ would be an intuitive and dynamic resource hub to help improve communication across services and improve access to community resources.

Solution

Based on this need and knowledge of the current resources available across the Integrated Care System, a community-focused HealthwaveHub has been implemented to help more people access the services they need.

Through active engagement strategies, Healthwave is fostering connections with community leaders to understand the challenges faced by their members and in turn empowering Navigators to create collections of national, local and hyper-local resources that can be easily shared with the people they are supporting.
